Save Time: Complete Forms Before Your Appointment
For all eye care appointments, we offer a patient portal for online forms. For patients that are new to the practice, you can access the patient portal and register a new account by clicking here. We ask that this process is completed prior to your scheduled appointment time. If you are unable to complete this ahead of time, we ask that you arrive 30 minutes early to fill out these forms on a tablet provided at the office. For established patients of the practice, you can login to your account to access medical records and update any information. For audiology/hearing appointments, the intake form can be downloaded to your computer and printed at home. In order for our doctors to provide a complete and comprehensive medical exam, it is important that your medical records are accurate and up to date. Please be prepared to provide this information to our office along with your current medical insurance information.
